CMS requires that all applicants must comply with the COVID-19 mandate for vaccination as of 11/5/2021.
An exemption form must be submitted to the Chief Compliance Officer in Administration Office for consideration for exemption approval, or you must provide a COVID Vaccination card for proof of vaccination.
The full COVID Vaccination policy will be available upon request. The CMS website for COVID mandate is https://www.federalregister.gov/documents/2021/11/05/2021-23831/medicare-and-medicaid-programs-omnibus-covid-19-health-care-staff-vaccination.
Please furnish one of the following when applying for a position with ACMC:
Proof of COVID-19 Vaccination
Medical Exemption Request form
Religious Exemption Request form
