Worker's Compensation
Notify your supervisor about all workplace injuries.
All injuries must be called into Medcor at:
1-800-775-5866
- If your injury needs immediate medical assistant please go to the ER Dept for care. If your injury is minor please go ahead and call Medcor at 1-800-775-5866 to report the injury. If you are unable to call, your supervisor may call to report the injury and then Medcor will contact you.
Information needed for Medcor call
- Full Name
- Work phone number
- Name of the company where you work
- Social Security or employee ID number
- Date of injury
- Medcor may give you treatment instructions to apply ice packs or other first aide remedies. They may even tell you to go to ER or ACMC Workman Comp doctor – Dr. Brad Walsh. Please follow all instructions.
- Employee must go to the lab to have a Drug test following an accident.
- Complete the Arkansas N form, print and send to HR Department.
- Complete an injury report form, print and send to Sarah Cope - Risk Management.
- Follow all physician orders and notify your Department Manager and HR of the status of your injury. If the physician takes you off work, please send the doctor excuse to HR Department.
